Medical Services & Clinical Decisions

Submitting an appointment request or participating in a consultation does not guarantee that any particular medication, treatment, laboratory test, diagnostic test, referral, or other service will be ordered or prescribed.

All clinical decisions are made by the treating licensed healthcare provider based on the patient's individual circumstances, medical history, clinical evaluation, applicable standards of care, and applicable federal and state law.

A provider may determine that additional medical records, laboratory testing, diagnostic testing, an in-person examination, specialist consultation, or other evaluation is necessary before initiating or continuing treatment.

Descriptions of services, including hormone therapy, medical weight-loss services, peptide-related wellness services, and other treatments offered by Beyond Health, are general in nature. Eligibility and medical appropriateness are determined individually following clinical evaluation.

Individual responses to medications, treatments, lifestyle interventions, and other healthcare services vary. No particular outcome, amount of weight loss, symptom improvement, aesthetic result, or other specific result is promised or guaranteed.

Testimonials, examples, or descriptions of patient experiences, if displayed on this website, should not be interpreted as a guarantee that another patient will experience the same or similar results.

Prescription Medications

Scheduling or completing an appointment does not guarantee that a prescription medication will be prescribed, continued, changed, or refilled. Prescription medications are provided only when an appropriately licensed healthcare provider determines that treatment is medically appropriate and permitted under applicable law.

Beyond Health providers may require follow-up appointments, laboratory testing, monitoring, medical records, or other evaluations as clinically appropriate before initiating or continuing prescription treatment.

Telehealth Services

Beyond Health may provide certain healthcare services through telehealth when clinically appropriate and legally permitted. Telehealth has benefits as well as limitations and does not always provide the same information available through an in-person physical examination.

Patients participating in telehealth services are subject to Beyond Health's separate Telehealth Consent, which explains the nature of telehealth, potential limitations and risks, patient responsibilities, privacy considerations, emergency procedures, and the patient's right to withdraw telehealth consent.

Billing, Insurance & Patient Responsibility

Insurance coverage and benefits vary by patient and health plan. Verification of insurance benefits or participation does not guarantee payment by an insurance company.

Patients remain responsible for applicable copayments, deductibles, coinsurance, non-covered services, self-pay charges, membership or program fees, and other patient-responsibility amounts in accordance with applicable Beyond Health financial policies. Please contact the office with questions regarding billing, insurance participation, or financial policies.

Privacy

Beyond Health respects the privacy and confidentiality of patient information. Our collection, use, and disclosure of protected health information are addressed in our Privacy Policy & Notice of Privacy Practices. Patients have rights concerning their protected health information as described in that Notice and under applicable law.

Electronic & SMS Communications

Beyond Health may offer telephone, email, patient portal, and SMS/text-message communications. SMS/text-message communications are governed by our separate SMS Terms & Consent. Consent to receive SMS communications may be withdrawn at any time as described in those SMS Terms. Electronic communications should not be used for medical emergencies.

Third-Party Links & Services

Links to appointment scheduling, patient portals, payment systems, telehealth platforms, and other external resources are provided for convenience. Beyond Health may use third-party technology providers to facilitate certain services. Third-party websites and services may maintain their own terms, privacy policies, and security practices.

Beyond Health is not responsible for the content, availability, privacy practices, or independent activities of unrelated third-party websites.

Website Use

You agree to use this website only for lawful purposes. You may not attempt to gain unauthorized access to patient information, accounts, servers, networks, databases, or other restricted systems; interfere with website operation or security; introduce malicious software; impersonate another individual; or use the website for fraudulent or unlawful activity.
